Your First Appointment
What to do:
- Contact your insurance company to determine if our services are covered and if you need a referral from your primary care provider. Please get this referral prior to your first visit.
- Please arrive 30 minutes before your scheduled appointment time to complete information forms.
- Please bring all insurance cards, a photo ID, and a current list of medications. Inform our medical staff of any known allergies.
- If you are unable to keep your appointment, please notify our office 24 hours before your appointment.
